Program Coordinator - Re-Entry Specialty MDT

  • Position Summary

    The AOD/SUD Counselor reports to the Clinical Coordinator and Peer Navigator Manager and is responsible for ensuring the effectiveness of the organization's recovery programs and partnerships, with a specific focus on youth and young adults (ages 12-24). This role oversees the day-to-day management of Recovery Programming, providing leadership to support its development and long-term sustainability while ensuring services are age-appropriate and responsive to the unique needs of adolescents and transitional-aged youth. Additionally, the AOD/SUD Counselor will play a key role in addressing program challenges and providing informed guidance to the Program Supervisors

    Essential Functions/Responsibilities

    • Will lead Project Kinship in recovery programming for youth and young adults (ages 12-24) struggling with substance use disorders and addictions.

    • Identify service gaps and client needs, providing feedback to the Management Team and Program Supervisors while collaborating on solutions.

    • Develop and maintain relationships with community substance use service providers, including but not limited to detox centers, residential treatment centers (RTC), partial hospitalization programs (PHP), intensive outpatient programs (IOP), outpatient programs (OP), methadone clinics, and medication-assisted treatment (MAT) providers.

    • Demonstrate knowledge and experience with Alcoholics Anonymous (AA), Narcotics Anonymous (NA), abstinence-based programs, harm reduction strategies, and SMART Recovery, with an understanding of age-appropriate approaches for youth.

    • Work both independently and collaboratively, demonstrating strong initiative and problem-solving skills when working with youth and young adults.

    • Develop and facilitate age-appropriate recovery-focused groups, workshops, and panels tailored to youth and young adult experiences.

    • Conduct initial risk assessments addressing clients’ physical, mental, emotional, and social needs, literacy and skill levels, substance use status, and recovery needs, ensuring that interventions are developmentally appropriate for youth and young adults.

    • Provide individual and small group counseling sessions focused on recovery, relapse prevention, self-help skills, problem-solving, and trigger management, specifically addressing challenges faced by youth and young adults.

    •  for clients and family members, and ongoing case management with clients post release from incarceration.

    • Engage clients in their assessment, evaluation, and treatment plan development to promote personal responsibility in their recovery, while collaborating with the multidisciplinary team (MDT).

    • Set measurable, time-bound short- and long-term goals in treatment plans and update them as needed to support youth and young adult clients’ progress.

    • Maintain complete, accurate, and timely documentation in case files, including assessments, treatment plan updates, group and individual session notes, incident reports, and all required documentation.

    • Monitor and document signs of substance use, relapse triggers, and crisis incidents, discussing concerns in case consultations with the Management Team and Program Supervisors.

    • Provide crisis intervention, de-escalation, and redirection as needed, engaging emergency professionals when required.

    • Support clients at their current stage of recovery and conduct weekly check-ins as determined by the treatment team and supervisors.

    • Facilitate connections with resource agencies that specialize in services for youth and young adults to help clients meet their treatment goals.

    • Ensure all documentation is updated in the Electronic Medical Record (EMR) system, while strictly adhering to confidentiality and HIPAA regulations.

    • Maintain an active caseload of up to 60 clients at any given time, with a primary focus on youth and young adults.

    • Present cases during weekly treatment plan review meetings with the Management Team and Program Supervisors.

    • Understand and support key re-entry priorities, including mental health services, substance use disorder treatment, diversion programs, housing assistance, case management, peer support, life skills training, job skills training, and civil legal services for youth and young adults in the criminal justice system.

    • Uphold HIPAA compliance, professional boundaries, and ethical standards when working with staff and clients.

    • Attend required trainings, submit timecards and leave requests on time, and complete assigned duties in accordance with deadlines.

    • Implement management decisions in a professional, timely, and positive manner.

    • Demonstrate commitment to the agency’s Mission, Vision, and Values, upholding its philosophy and objectives.

    • Work a flexible schedule, including evenings and weekends, as needed to support the program and leadership.

    • Compile and submit required reports, including monthly units of service, demographics, and evaluation data, to the Data Analyst and Program Supervisors in a timely manner.

    • Perform other duties as assigned by the agency.

    Education and Experience

    • Minimum of High School Diploma or equivalency

    • Certified Substance Use Counselor (CADC, RAD-T, etc.) must be current and able to show proof of degree/certification OR with RAC registered number and shows proof of being active in education program towards pursuit of certification AND minimum of 3 years of experience in substance use counseling.

    • Minimum of 2 years of experience working with youth and young adults (ages 12-24), preferably in a counseling, case management, or mentorship capacity.

    • Experience working with system impacted youth, and/or gang-involved individuals is highly preferred.

    • Experience working as part of a treatment team in service of dual-diagnosis, gang-involved, incarcerated or formerly incarcerated individuals is highly preferred.

    • Bilingual in Spanish or other threshold languages is preferred.

    Other Qualifications

    • Must complete a Live Scan Background Check.

    • Must possess knowledge of: 12 Step recovery curriculums; drug and alcohol addiction; co-occurring substance abuse and mental health disorders; withdrawal processes and symptoms; relapse prevention strategies; trigger behavior identification; family system dynamics; the impact of trauma; and common obstacles clients encounter at all stages of recovery.

    • Able to recognize individuals under the influence, at-risk for relapse, or at-risk for harm to self or others.

    • Ability to work and communicate effectively as part of a treatment team consisting of staff, family members, law enforcement and court officials, service providers, and community members.

    • Extensive knowledge of social service, welfare, legal, corrections, health care, substance abuse, mental health care, and other systems associated with the target population.

    • Will oversee administration of staff and provide clinical consultation and coordination with team.

    • Will assist Program Manager to support collaboration of services between programs and teams.

    • Will assist Program Manager with submissions of monthly reports and program narrative to ensure best practices and compliance of the program, focused on clinical soundness.

    • Participate in staff meetings, trainings, case conferences, team meetings, and supervision sessions that cover all stages of treatment, share best practices, and ensure standard compliance.

    • Work closely with Clinical Case Managers, Case Managers, Peer Navigators, Clinician and Substance Use Counselors to ensure client referrals, linkages and or treatment plans are being followed.

    • Collaborate effectively with community organizations, county services, school districts, and wrap around services to establish a continuity of care for "live handoff” linkages for clients.

    • Maintain case files in a ready state for audits according to quality assurance and compliance standards.

    • Ensure mental health treatment services are provided in concert with the policies, procedures, government rules, and regulations for the program.

    • Participate in trainings, perform assigned duties, and submit timecards/leave requests according to deadlines.

    • Ensure HIPAA, ethical code of conduct, and professional boundaries are maintained with staff and clients.

    • Implement management decisions in a positive and timely manner.

Priorities are to provide appropriate mental health services, substance use disorder treatment assessment and “live linkage” to diversion/residential programs for individuals in the criminal justice system; provide linkage and support to housing-related assistance, case management, peer support, recovery services, life skills, job skills training and civil legal services.

    • Work a flexible schedule outside of business hours, including weekends and evening hours, as the need arises or to support the program and leadership.

    • Compile and submit weekly units of service, demographics, and other required evaluation data to the Data Analyst and Program Manager in a timely manner.

    Education and Experience

    • Master’s Degree in social work or related field, must be LCSW/LMFT License eligible, within six months of the required hour completion, and qualified to take the LCSW/LMFT Licensure Exam within one year.

    • At least 2 years of leadership experience with at least 1 year directing/supervising staff in the re-entry field.

    • At least 3 years of experience working within mental health programs, especially those that work with gang involved, incarcerated, recovery, severe mental illness, and/or re-entry populations and their families is required.

    • Minimum of 3 years of experience providing direct linkage and support to clinical counseling, case management, and knowledge of rehabilitation philosophy and trauma-informed best practices is required.

    • Qualified to provide supervision for Masters and Bachelors Level Social Work Interns is preferred.

    • Knowledge of DSM-V and working principles of recovery with adults.

    • Must complete a Live Scan Background Check.

    • Experience in working within county contract reporting requirements is preferred.

    • Intermediate PC skills required; experience in working with Electronic Medical Record systems preferred.
