Restorative Practices Intervention Specialist

Youth Transformation Program

  • Essential Functions/Responsibilities

    • General Responsibilities

      • Commit to the agency’s Mission, Vision, and Values.

    • Circle Facilitation & Support

      • Plan and coordinate logistics for Circle practices, including pre-conferences for Re-Entry and Support circles.

      • Provide follow-up with students and staff to address circle-related incidents.

      • Develop tools for best practices, such as templates, videos, and presentations.

      • Lead professional development workshops for staff and students.

    • Intervention & Case Management

      • Lead interventions for at-risk students and families, focusing on support, mentoring, life skills, and coaching.

      • Provide mediation services for behaviors like bullying and aggression.

      • Offer intensive case management, including referrals and access to care for students and families.

    • Documentation & Compliance

      • Maintain accurate documentation (Apricot, on-site records, sign-in sheets) in compliance with quality assurance standards.

    • Collaboration & Continuity of Care

      • Collaborate with school personnel, district officials, community organizations, and county services to ensure culturally relevant referrals and continuity of care.

      • Attend relevant meetings (COST, school site, district, agency-wide) and training at Project Kinship and other sites.

    Education and Experience

    • Bachelor’s degree or equivalent experience

    • At least one year of Restorative Practices and Violence Prevention experience

    • Preferred experience working with system impacted/gang involved youth

Program Coordinator - Re-Entry Specialty MDT

Schools

  • Essential Functions/Responsibilities

    • Will lead Project Kinship in recovery programming for those struggling with substance use disorders and addictions.

    • Will recognize needs and gaps in service for population and will provide feedback to Clinical Coordinator, and program leadership and will collaborate in problem solving.

    • Will build relationships with substance use related service providers in the community, including but not limited to: Detox centers, RTC, PHP, IOP, OP, methadone clinics, MAT, and others.

    • Must be experienced with AA/NA, Abstinence based programs, Harm Reduction & Smart Recovery.

    • Must be a self-starter and comfortable working both independently as well as part of a team.

    • Must be able to build and run recovery-focused programming to include groups, workshops and panels.

    • Conduct initial risk assessment on client’s: physical, mental, emotional, and social needs; skill and literacy level; substance use status; recovery needs; and appropriate levels of care.

    • Conduct individual and small group recovery-focused counseling sessions; workshops on relapse prevention, self-help skills, problem solving, and trigger behavior management with clients and family members; and ongoing case management with clients post release from incarceration.

    • Include clients in their own assessment, evaluation, and treatment plan development to cultivate a sense of responsibility in their own recovery.

    • Work with the multidisciplinary team to create individualized treatment plans which will include short and long-term goals, that are measurable, time bound, reviewed, and updated as needed to successfully progress clients’ treatment plans.

    • Document assessment results, treatment plan updates, group notes, individual session notes, incident reports, and all required documentation in case files in a complete, accurate, and timely manner.

    · Document signs and symptoms of use, relapse triggers, and crisis incidents in client’s file and discuss during case consultation with the Clinical Coordinator.

    • Redirect, de-escalate, and intervene during crisis situations, when appropriate; engage emergency professionals immediately when needed.

    • Network with resource agencies to facilitate “warm handoff” linkages to meet treatment goals.

    • Maintain accurate and up to date information in each client’s chart, document client contact by using the EMR (Electronic Medical Record) system, and adhere to confidentiality practices.

    • Maintain an ongoing client caseload of up to 60 clients at any given time.

    • Present cases during weekly treatment plan review meetings with Program Manager and clinical staff.

    · Will have a broad understanding of re-entry services and behavioral health services, priorities are to provide mental health services, substance use disorder treatment and diversion programs for individuals in the criminal justice system; provide housing-related assistance, case management, peer support, recovery services, life skills, job skills training and civil legal services.

    • Ensure HIPAA, ethical code of conduct, and professional boundaries are maintained with staff and clients.

    • Participate in trainings, perform assign duties, and submit timecards/leave requests according to deadlines.

    • Implement management decisions in a positive and timely manner.

    • Commit to the agency’s Mission, Vision, and Values; represent actions that define the overall philosophy and objectives of the agency.

    Education and Experience

    • Minimum of High School Diploma or equivalency

    • Certified Substance Use Counselor (CADC, RAD-T, etc.) must be current and able to show proof of degree/certification and minimum of 3 years of experience in substance use counseling.

    • Experience working as part of treatment team in service of dual-diagnosis, gang-involved, incarcerated or formerly incarcerated individuals is highly preferred.

    • Bilingual in Spanish or other threshold languages is preferred.

    Other Qualifications

    • Must complete a Live Scan Background Check.

    • Must possess knowledge of: 12 Step recovery curriculums; drug and alcohol addiction; co-occurring substance abuse and mental health disorders; withdrawal processes and symptoms; relapse prevention strategies; trigger behavior identification; family system dynamics; the impact of trauma; and common obstacles clients encounter at all stages of recovery.

Recovery Specialist

Re-Entry Success Center

  • Essential Functions/Responsibilities

    • Will oversee the day-to-day aspects of the program, including eligibility of program, triage, supportive service requests and overall daily support of the team.

    • Will oversee administration of staff and provide clinical consultation and coordination with team.

    • Will assist Program Manager to support collaboration of services between programs and teams.

    • Will assist Program Manager with submissions of monthly reports and program narrative to ensure best practices and compliance of the program, focused on clinical soundness.

    • Participate in staff meetings, trainings, case conferences, team meetings, and supervision sessions that cover all stages of treatment, share best practices, and ensure standard compliance.

    • Work closely with Clinical Case Managers, Case Managers, Peer Navigators, Clinician and Substance Use Counselors to ensure client referrals, linkages and or treatment plans are being followed.

    • Collaborate effectively with community organizations, county services, school districts, and wrap around services to establish a continuity of care for "live handoff” linkages for clients.

    • Maintain case files in a ready state for audits according to quality assurance and compliance standards.

    • Ensure mental health treatment services are provided in concert with the policies, procedures, government rules, and regulations for the program.

    • Participate in trainings, perform assigned duties, and submit timecards/leave requests according to deadlines.

Priorities are to provide appropriate mental health services, substance use disorder treatment assessment and “live linkage” to diversion/residential programs for individuals in the criminal justice system; provide linkage and support to housing-related assistance, case management, peer support, recovery services, life skills, job skills training and civil legal services.

    • At least 2 years of leadership experience with at least 1 year directing/supervising staff in the re-entry field.

    • At least 3 years of experience working within mental health programs, especially those that work with gang involved, incarcerated, recovery, severe mental illness, and/or re-entry populations and their families is required.

    • Minimum of 3 years of experience providing direct linkage and support to clinical counseling, case management, and knowledge of rehabilitation philosophy and trauma-informed best practices is required.

    • Qualified to provide supervision for Masters and Bachelors Level Social Work Interns is preferred.

    • Knowledge of DSM-V and working principles of recovery with adults.

    • Must complete a Live Scan Background Check.

    • Experience in working within county contract reporting requirements is preferred.

    • Intermediate PC skills required; experience in working with Electronic Medical Record systems preferred.

    • Bilingual in Spanish or other threshold languages is preferred.

Chief Financial Officer

  • Essential Functions/Responsibilities

    • Develop and implement financial strategies to support company goals.

    • Oversee budgeting, forecasting, and financial reporting processes.

    • Manage cash flow and working capital to ensure operational efficiency.

    • Responsible for ensuring the financial accounting procedures manual is up to date and consistent with best practices.

    • Lead annual external audit processes of the C3; prepare and support audits from other agencies as contractually required

    • Interact and advise the Executive Team on matters of tax, external regulatory reporting, and budget development and reporting, both annual and monthly performance reporting.

    Minimum Education

    • Bachelor’s degree in Accounting, Finance or related field required.

    • Master’s degree in Accounting or CPA is a plus.

    Minimum Work Experience

    • A minimum of 7 years of previous hands-on accounting, financial reporting and analysis experience.

    • 5-10 years of progressive management experience in finance or accounting, ideally within a nonprofit of similar size and structure.

    • Relevant understanding and experience with US GAAP
